He was involved in a heavy clash of heads with Exeter goalkeeper Joe Whitworth that left him with a bloody nose and carried off on a stretcher.
Awoniyi had scored to put the Reds ahead in their FA Cup tie to make it 2-1 before the equaliser by the Championship side.
The encounter progressed to penalties at 2-2 after extra time before Forest won 2-4 to reach the 5th round of the competition.
